In my exploration of spinal health, I’ve unveiled the critical role of spinal marbling—fat deposition in the muscles of the spine—as a significant marker of injury, particularly following a whiplash event. Over three decades of research have led me to understand the cascade of events that begins with muscle spasm and progresses to the persistent compression of spinal elements. This sequence significantly alters the spine’s anatomy, leading to chronic conditions that traditional imaging techniques often miss. My development of spinal botulinum toxin and PRP injection methods over 20 years ago was aimed at combating this cycle by assisting in the remodeling of spinal elements and soft tissue. This approach has brought relief to many patients facing catastrophic pain syndromes, highlighting the potential for recovery even in the most severe cases.

I will further explain why spinal marbling is critical in injury assessment.

What I have learned, from 3 decades of spinal research, is that the whiplash event causes profound injury of the intrinsic muscles of the spine, which triggers a persistent spastic event of these muscles.

The spasm then causes persistent compression of some spinal elements which alters the very innervation of these intrinsic muscles, which then causes further compression of the spine and nerve root with partial paresis of these spinal muscles on an ongoing circuit.

Over time, let’s say 3 months, the spastic, dystonic spinal muscle develops massive levels of high-density scarring which immobilizes the spinal segment or segments, and causes overloading and compression of disks, entrapping nerves, or parts of a nerve, and altering the normal spinal anatomy, possibly affecting the passage of even the spinal cord centrally along the vertebrae.

This persistent spasm with post injury scarring also immobilizes the deep spinal muscle allowing for wasting or fat to form. Further a branch of the dorsal nerve supplying the intrinsic muscle is often affected contributing to what I call a dystonic-like spasm creating a vicious circle of injury followed by spasm followed by scarring followed by more nerve impairment followed by more spasm, and the circuit continues.

It was for this reason, over 20 years ago, I developed and patented spinal botulinum toxin and prp injection methods with the intention of assisting in the remodelling of the spinal elements and soft tissue to reverse this process.

I have treated many patients over 30 years with good and great success in what would be labelled catastrophic pain syndromes affecting the spine.
